The Difference Between Luer Lock and Slip Tip Syringes

Luer lock and luer slip are the two main connection systems used to attach needles to syringes. The difference between them is the security of the needle-to-syringe connection. Choosing the wrong type for a specific application can result in needle disconnection during injection, which is both a medication error and a sharps safety incident. This guide explains how each type works and when each should be used.
A luer slip syringe has a plain tapered nozzle that pushes into the corresponding tapered hub of the needle. The connection is held by friction between the two mating tapers. Assembly is quick and requires no twisting: the needle hub simply pushes onto the syringe nozzle. Luer slip is suitable for most routine low-pressure subcutaneous and intramuscular injections in a controlled setting, where the injection pressure is modest and the risk of accidental disconnection during administration is low.
A luer lock syringe has a threaded collar on the nozzle and the needle hub has a corresponding internal thread. When the needle is attached, the collar is twisted to engage the threads, locking the needle onto the syringe in a connection that cannot be pulled apart by axial force. Releasing the needle requires deliberate counter-rotation of the collar. Luer lock provides a secure connection that resists accidental separation during high-pressure injections, through extension lines, or during administration of viscous preparations.
All standard clinical luer components are manufactured to ISO 594, which specifies the taper geometry and dimensions. This means any ISO-compliant luer slip needle fits any ISO-compliant luer slip syringe, and any luer lock needle fits any luer lock syringe. Cross-matching a luer slip needle on a luer lock syringe provides only friction connection without any locking benefit. Using a luer lock needle on a luer slip syringe similarly provides no locking advantage. When luer lock security is required, both the syringe nozzle and the needle hub must be luer lock.
