Luer Lock or Luer Slip: Which Connection Is Best for Me

Luer lock and luer slip are the two standard needle-to-syringe connection systems. Both use the same ISO 594 taper geometry and are universally cross-compatible within connection type. The difference is security: luer slip connects by friction and can be pulled apart; luer lock threads the needle hub onto the syringe nozzle and cannot be accidentally disconnected. Choosing between them comes down to the pressure, the route, and the clinical setting.
A luer slip needle hub is a smooth internal taper that pushes onto the matching external taper of the syringe nozzle. Friction between the two surfaces holds the connection. Assembly is fast: push and twist slightly to seat. The connection holds well under the modest plunger pressures of most subcutaneous and intramuscular injections in a controlled setting. It can, however, be pulled apart by axial force if tension is placed on the needle during administration or if high plunger pressure is applied with a viscous preparation.
A luer lock needle hub adds a threaded collar that screws onto a matching thread on the syringe nozzle. Once engaged, the connection cannot be separated by axial force; deliberate reverse rotation of the collar is required to release the needle. Luer lock takes marginally longer to assemble than luer slip but provides a secure connection that will not disengage under high plunger pressure, patient movement, or tension from extension lines.
All standard clinical luer components comply with ISO 594, which specifies the taper angle and dimensions. This means any luer slip needle fits any luer slip syringe, and any luer lock needle fits any luer lock syringe, regardless of brand. Cross-matching a luer lock needle to a luer slip syringe provides only friction connection without locking benefit. For luer lock security, both the syringe nozzle and the needle hub must be luer lock.
